Services Provided:

  • Provide care coordination services to high-risk pregnant, postpartum women and their infants.
  • Conduct risk assessment, develop a care plan, and make appropriate referrals and follow-ups.
  • Work as a liaison with medical, mental health, and social service agencies to meet client needs.
  • Provide one-on-one education and group education to clients per program protocol.
  • Conduct home and virtual visitation and monitor family’s safety.
  • Maintain client charts per program protocol and gather data as requested for research or reporting.
